Breast Cancer Reconstruction After Nipple Sparing Mastectomies: This young lady in her early 30s had bilateral nipple sparing mastectomies for breast cancer and was BRCA gene positive. She also had a lymph node biopsy. I performed a 2 stage breast reconstruction using tissue expanders initially at the same time as the mastectomies. A breast fold incision was used. I spend the time with the surgical oncologist to help create the safest and best aesthetic scar option. I also try to help preserve blood supply to the nipple, when it can be saved. A little over 3 months later, the tissue expanders were exchanged for 650 cc silicone gel “gummy bear” implants. Here she is 7 weeks after that second operation. I think a better result can often be obtained with a 2 stage surgery. Also, this patient desired to have slightly larger breasts than a one stage procedure would allow. Sometimes nipple sparing procedures are “more safely” performed using tissue expanders, due to the nipple blood supply. These photos show both the before mastectomy and the intermittent stage with larger tissue expanders on the left side. The final result is alway on the right in these photos. The patient desired slightly larger breasts, so I over expanded slightly in the intermediate stage.
